Parish Registration

You can request a registration packet in one of three ways:

• Visit the parish office and pick up a packet.
• Call the parish office at (602) 867-9171 and ask for a packet to be mailed to you.
• Fill out the following on-line form, and a packet will be mailed to you.

After you have completed the packet, it may be mailed or dropped off at the parish office or dropped in the collection basket at Mass.
